COVID-19 Infection Survey Digital - User Feedback

Overview

This survey aims to understand your experience of participating in the COVID-19 Infection Survey (CIS) and identify potential areas for improvement. It should take around 5 - 10 minutes to complete and can be completed by anyone taking part in the online and telephone questionnaire and postal and courier sample returns approach to the CIS about their most recent testing window. This is different to when study workers used to visit your home to do the questionnaire and collect the samples. You will not be able to provide feedback about a face-to-face appointment in this survey.  

It can also be completed more than once, if you wish to complete it again at a later date to reflect a more recent experience.   

Please note that this survey is completely independent of the CIS, so your data from the CIS will not be linked with the answers you give here.  Responses to this survey are anonymous, and therefore it will not be possible to retrospectively remove any responses once they have been submitted. If you choose to request a pdf copy of your response by email at the end of the survey, your email address will not be stored.
